Saint Peter’s University Hospital Community Health Services will be coming to ACI to provide flu vaccines.

Please bring to the appointment:

• Primary and secondary insurance cards

• A photo identification card

• The completed Influenza Vaccine Clinic Registration form

Appointments are required. Walk-ins may be accommodated.
